AC Diagnostics welcomes the opportunity to investigate all complaints received in writing from an identified source, as they may contribute to opportunities for improvement. AC Diagnostics reviews complaints against its own accreditation services as well those against facilities accredited through our programs. Only complaints that are received in writing or on this form, with the identification of the complainant, are investigated. We ask that you please provide a detailed description below when submitting your complaint. If this complaint is being filed on behalf of a facility, the complainant’s facility name, AC Diagnostics site number and the licence number (for mandatory Ontario Laboratory Clients only) must be provided, in addition to the complainant’s contact information. If you are writing to file a complaint against an accredited facility, please ensure that you follow the facility’s complaint process first. Note: The form below is for complaints only. If you wish to appeal an accreditation decision, please contact your assigned staff member from the AC Diagnostics team and they would be able to assist you
