The third phase—Maintaining Accreditation—is under development and will included full re-assessments (repeat Phase 2) and surveillance activities on a schedule aligned to certificate expiry dates.  

Re-assessments

Beginning in mid-2020, a process will be implemented to repeat the process for on-site review/case review as described in Phase 2 for all accredited facilities. If the facility is successful, the re-assessment will conclude with the issuance of a new accreditation certificate. This cycle for on-site reviews repeats in alignment with certificate expiry dates.
