Phase 2 — Step 3: Pre-assessment Information and Case Studies


Notification of assessment occurs about four months of the expected date of on-site review. Facilities are provided with a Pre-visit Questionnaire and are asked to share selected documentation in advance of the assessment.

EQI Program staff will also provide criteria for the selection on case reviews to be assessed for quality and completeness of the images and the completeness, consistency and quality of the final reports (TTE, SE and/or TEE studies). The number of studies requested depends on the number of physicians and modalities offered at the facility, and could date back six months. We request random studies (TTE, SE, and/or TEE images), both with and without pathology.
