Once the declaration has been reviewed and the facility has been deemed to meet the requirements for EQI Provisional Recognition, the EQI Program will notify both the facility and Ontario Ministry of Health (MOH). A letter of provisional recognition will be provided to the facility; a copy of this letter must be submitted to the MOH with your group billing number application to receive echocardiography OHIP billing privileges.
The facility will then proceed to Phase 2 — Step 3: Pre-assessment Information and Case Studies.
